Checklist instructions for the OMR system
Before the test/examination
- Obtain the correct candidate answer forms for students to use. Currently, we are using the GPAS-240 standard form [JPG 249kb]. You must use the correct form - don't use the sample images from this website, they won't work! Correct forms are obtained from your Faculty office.
- Download the OMR Request Form [DOC 75kb] from this website. Complete the form and select the reports you wish to have returned to you. (See the Samples of reports page to help you decide which ones you want.)
- Using one of the GPAS-240 candidate answer sheets, carefully shade to indicate the correct answers. Student papers will be compared to this answer key. Errors here can have a disastrous effect on student marks, so check this carefully! Shade 00000000 in the student ID box to indicate that this is the answer key. (Do not mark the answer key in any other area).
At the test/examination
- Ensure that students are correctly briefed on the correct way to fill in the answer sheet. We have developed a Briefing Sheet [DOC 55kb] available on this website to help you catch the most common errors students make.
After the test/examination
- Collect the completed candidate answer sheets and submit them to the Reception office of the Office of Assessment, Teaching & Learning, Bentley Campus Bldg 105.136 (ground level of Bldg 105 directly below the entrance to the library); (please do not send via mail). Make sure that the pack you submit includes:
- The OMR Request Form, completed as required;
- The answer key with 00000000 entered as student ID;
- The stack of completed candidate answer sheets.
- OTL will process the sheets and return the requested results to you by email. This will include feedback on any issues or problems detected.
- The processed candidate answer sheets will be returned to you in the internal mail, or you can elect to collect them in person if you have specified this on the request form.
